Team Up Derbyshire is a service where health and social care professionals work together to support people in their own homes, particularly those who find it difficult to get out to appointments.
What does it do?

The team provides care at home, so patients don’t always need to go to hospital or the GP surgery
It offers:
- Planned and routine care
- Urgent support when someone becomes unwell
- Help to prevent problems before they happen
Who is it for?
- People who are housebound or very frail

- Patients who need extra support to stay safe and independent at home

How does your GP surgery fit in?
Manor House Surgery will remain the main point of contact for your care.
Your GP and practice team are part of the Team Up network, working alongside community and council services
If needed, the GP surgery can refer you into the Team Up service
Your GP will continue to:
- Oversee your overall care
- Share information with the Team Up team
- Help coordinate the support you receive
The Team Up team may then:
- Visit you at home
- Arrange additional support (for example nursing, therapy, or social care)
- Work with your GP to make sure your care is joined up and personalised
How does it help patients?
