Social prescribing is when health professionals refer patients to receive support in the community.  This support helps to improve their health and wellbeing and the concept has gained support within many NHS organisations across the country.

 GPs and other frontline healthcare professionals are able to refer patients to a link worker. This provides them with a face to face conversation where they can learn about wider possibilities and receive support to design their own personalised solutions, i.e. ‘co-produce’ their ‘social prescription’- so that people with social, emotional or practical needs are empowered to find solutions which will improve their health and wellbeing.  Many of these services are provided by the voluntary and community sector.

 Social Prescribing is an innovative and growing movement, with the potential to reduce the financial burden on the NHS and particularly and ease the ever increasing pressures on general practice.
